Noticeboard

In May 2017 we joined with the six other practices in North Ceredigion, the new name of the limited company is Cambrian Primary care. We hope to work more together in the future to enable services to continue being delivered efficiently and effectively. Limited data may be shared amongst practices in the future - for more information or to opt out see 'Sharing of GP Record.'  For now we will all continue running as six individual practices as usual. By working together more we hope to sustain and improve the services we currently provide.

To refer yourself simply fill out the front and back pages of the self referral form. Once you have filled this out, send the referral to one of the addresses on the bottom of the self referral form.

The physiotherapy department says "We aim to see urgent referrals within two weeks and routine referrals within 14 weeks of receipt of the form. If you are placed on the urgent list, a letter will be sent to you immediately asking for you to call the department to make a convenient appointment. If you are placed in a routine waiting list, you will be sent a letter that will ask you to phone and make an appointment once you are near the top of that list. If we have no contact from you within a month of the letter having been sent, you will be discharged. A further letter will then be sent to you to confirm that you have been discharged."
